****Please note that the material you need in order to complete this assignment will be covered over the next few weeks. You may not be able to fully complete this assignment right away as we will be learning a lot of this information during this section. It is due Friday, April 28, 2023 at 11:59pm.
Step 1: Find a friend or family member to interview. Ask them to make up a patient who is struggling with a mental health problem.
Step 2: Use the “Intake Assessment Form” on pages 460-467 in the hardcopy book to guide your questions. This is for you to take notes on but you will NOT turn them in. You will take them and then write a fabricated intake report in paragraph form. You can find this in the electronic book by opening the platform and clicking on the search bar in the upper right-hand corner. You will be able to search by page using this feature.
Step 3: Using Word, you will write a full intake report.
A. Place a “Header” which states “FABRICATE REPORT BASED UPON FICTICTIOUS INFORMATION.”
B. Write a report in full paragraph form, detailing the information you learned from your interview. There is an example on pages 307-309 in your hard copy book. This is called a Social History or “Intake” Assessment. You should include information about childhood and family history, medical history, social history, employment history, summary of symptoms/history of the mental health concern, drug and illicit substance use, suicide risk, homicide risk, a mental status review, etc. End your report with a summary and recommendations for treatment. Your final report should be between three and five pages. I am not worried about length but about capturing all necessary information.

Name or Title of the strategy (For example- Jigsaw) 1 pt. 2. Researcher/ Founder-If a specific person is associated with this strategy, give that name, the name of book or article, and page no. (For example, Gayle Gregory and Carolyn Chapman, Differentiated Instructional Strategies, (p.70). 1 pt. If the information comes from your text book you may even give the page number for later reference. I am emphasizing the researchers in the field of D. I. because I want you to be more than familiar with these researchers and their contributions. You have encountered many of them after reading your text book. Learn who they are- starting with the two authors of your text and be able to tell of their contributions to the field. You have comps coming up and this is the kind of information that you may need to know to pass the test. 3. Explanation-I will need a FULL explanation of the method/strategy. 4 pts. 4. Use in my Subject Area- Detailed explanation of How you will use it in your classroom. 4 Pts.
